Please fill out and submit the prayer request form to add names to The Church . Names on the list will appear in the bulletin insert each Sunday. Names must be submitted by the Tuesday before the Sunday you would like them to appear in the bulletin.
In Name,Write Name for the Prayer List ,In subject write Reasons for Prayers,
In message box write choices that apply e.g. Healing, Other Needs and Concerns, Recently Departed etc. and How long should the name remain on the list? Two Weeks, One Month, Until Notified etc. and your name , phone number.
